Prior to your first therapy visit from Helping Hands, please be sure you have completed the following:

1. Gather any previous evaluations or information that would be helpful to the therapist.

2. Obtain original prescription (faxed or photocopied prescriptions cannot be accepted) from your child's pediatrician that states:

Your child's name and date of birth
Your child's diagnosis and diagnosis code(s)
Therapy requested
Frequency of treatment (e.g. Occupational Therapy two times per week)

3. Print out and complete the following five registration forms.

Intake
Billing Itake
Initial Consent
Notice of Privacy Practices
Consent to Treat Without Parent/Guardian

We must receive an original prescription for therapy and all five registration forms at least two business days prior to your first therapy visit . This ensures that we are able to verify insurance benefits and obtain prior authorization as necessary. In addition, this information will help your child's therapist be better prepared for their first therapy visit.
